The SFN 674 is to be completed by the HSZ case worker and reviewed and signed by both the supervisor and the agency director prior to submission to the Case Management FSS Unit (cmunit@nd.gov)

 

The case worker also is required to submit the following supporting documentation:

 

Required: SFN 674, SFN 824, SFN 1865, and if applicable the Children’s Treatment Services Level of Care Determination, PCPA, and Genogram.

 

Additional: a recent psychiatric evaluation, therapy notes, diagnostic evaluation, treatment plan, specialist evaluation, IQ Testing, IEP, police reports, etc.

 

It is important that each section of the referral is completed with as much detail as possible, and all supporting information is included for timely review. Incomplete referrals will not be accepted.

 

If approved for Comprehensive Case Coordination a Discovery meeting with the zone director, supervisor, case worker, and current CM FSS will be scheduled to discuss next steps. The custodial agency will continue to remain actively involved in continued planning as they will retain care, custody, and control of the child.
